Wellness Access Initiative

What is it?

At Halcyon House, we acknowledge that having much needed treatment can be costly.  Because of this, and through the generosity of donors, we are able to offer treatment subsidies for more affordable care.  An independent panel will meet to review the applications once a month and will determine the amount of financial assistance that you will receive.  Please fill this form out to the best of your ability.  We will not require any supporting documents but please be as truthful and accurate as possible. 

We divide our treatment assistance into 5 categories based on a percentage of the federal poverty guideline.

Level:

  • One covers 90% of the costs of your sessions
  • Two covers 70% of the costs of your sessions
  • Three covers 50% of the costs of your sessions
  • Four covers 30% of the costs of your sessions
  • Five covers 10% of the costs of your sessions

Donor Information

The Halcyon House Wellness Access Initiative is a community funded program to help offset the costs of our services for the people who need them the most. Selection for the program is based on financial need, physical need, treatment goals, and recommended individualized treatment plan from our staff. 

Thank you for your interest in supporting our community and helping to make affordable care available to everyone! We believe that the wellness services we provide are beneficial and even a potentially necessary part of healing and wellbeing. It is our hope that this wellness subsidy initiative will provide an avenue for access to our services for the people who need them the most but may need help to offset the cost.

Selection Committee

The selection committee is responsible for approval of applicants and management of Wellness Access Initiative funds. The committee composed of three to five individuals with at least one member-owner from Halcyon House’s Board of Directors. We ideally would like the majority of this panel to be composed of independent community members and donors. The member-owner from Halcyon House is a liaison between the selection committee and the Board of Directors and is responsible for assembling and de-identifying information about applicants to ensure HIPAA compliance.

How does the selection process work?

Applicants will fill out an online application to collect information about financial need, physical limitations/impairments and psychosocial concerns.  They will also have at least one session with one of the Halcyon team members who will report on their need for services (and which one/s), frequency and duration.  This information will be provided to the selection committee.

The selection committee is composed of three to five individuals with at least one member-owner from Halcyon House. We ideally would like the majority of this panel to be composed of independent community members and donors. The member-owner from Halcyon House is a liaison between the selection committee and the cooperative, and is responsible for making sure all identifying information about applicants is removed and all information available for the committee to access is HIPAA compliant.  

We will use this information and these tiers to decide the amount subsidized for your affordable care.

How does the selection committee determine which tier applies to you?

Level:

  • 1 is our highest award level, and the Wellness Access Initiative covers 90% of the cost of treatment. The financial guidelines are up to 150% of the Federal Poverty Limit ($48,225 for a family of 4).
  • 2 70% of treatment covered with financial guidelines of up to 175% of FPL.  
  • 3 50% of treatment covered with financial guidelines of up to 200% of FPL. 
  • 4 30% of treatment covered with financial guidelines of up to 225% of FPL. 
  • 5 10% of treatment covered with financial guidelines of up to 250% of FPL.
